rear wheel bearing

has anyone had the same problem im having at the min i had the back driver side wheel bearing change there on the 24th of august wasnt changed since new now today the replacement one has gone only about 4weeks in the car??? very p'd off bout it cos it cost 190 the first time to fix then 50 the second as the bearing was covered under warrenty anyone have any input on this
 
Hub itself where bearing goes damaged maybe? Where you driving around for a long period with bearing gone the first time?
 
From above has had x2 failures with new bearings. Or at some stage has wheel. had a knock that coulda damaged the hub?
